? Indications for Use
Janumet XR is a prescription tablet containing two active ingredients: Sitagliptin phosphate (a DPP-4 inhibitor) and Metformin hydrochloride (a biguanide). These two work together to control blood sugar levels in patients with Type 2 diabetes mellitus (T2DM).
Your doctor prescribed Janumet XR to help lower your high blood sugar levels when diet and exercise alone are not sufficient. It improves post-meal insulin levels and reduces sugar production by the liver, helping your body respond better to insulin.
? Contraindications
Do not take Janumet XR if you:
- Are allergic to any of the active or inactive ingredients.
- Have Type 1 diabetes or diabetic ketoacidosis (increased ketones in blood or urine).
- Have severe kidney impairment.
Avoid Janumet XR if you have conditions that may worsen kidney function, such as:
- Certain heart diseases or serious infections (e.g., fever, diarrhea, or vomiting causing dehydration).
- Undergoing X-ray procedures with contrast dyes.
Consult your doctor about when to stop and resume Janumet XR under these conditions.
⚠ Warnings and Precautions
Tell your doctor if you have:
- Kidney or liver disease.
- Heart problems, including congestive heart failure.
- Heavy alcohol consumption (regular or binge drinking).
- Pregnancy or plans to become pregnant.
- Breastfeeding.
- Any condition causing dehydration (severe vomiting, diarrhea, or fever).
? Pancreatitis Risk: Cases of pancreatitis (inflammation of the pancreas) have been reported. Stop Janumet XR and seek medical attention if you experience severe, persistent stomach pain (with or without vomiting).
? Serious Skin Reactions: Some patients may develop bullous pemphigoid (severe skin reaction) requiring hospitalization. Consult your doctor if you notice blisters or skin peeling.
⚠ Use in Children and Elderly
- Not recommended for patients under 18 years.
- Elderly patients should take Janumet XR under regular medical supervision.
? Dosage and Administration
- Take Janumet XR once daily with the evening meal to reduce stomach upset.
- Swallow tablets whole (do not crush, cut, or chew).
- If you see a tablet-like substance in your stool, inform your doctor.
- Do not stop taking Janumet XR without consulting your doctor.
⚠ Missed Dose:
- Take it as soon as you remember.
- If it is almost time for your next dose, skip the missed dose. Do not double the dose.
⚠ Overdose:
- Contact your doctor immediately if you take more than the prescribed dose.

- Severe kidney or liver disease.
- Heart failure requiring medication.
- Heavy alcohol consumption.
- Severe dehydration or undergoing surgery/X-ray procedures.

- Headache
- Hypoglycemia (low blood sugar)
- Gastrointestinal issues (diarrhea, nausea, bloating, vomiting, metallic taste, stomach pain)
- Upper respiratory infections (sore throat, nasal congestion)
Other Reported Side Effects
- Skin reactions (rash, hives, swelling of face/lips/tongue, difficulty breathing).
- Joint and muscle pain.
- Kidney problems (sometimes requiring dialysis).
- Blistering skin reactions.
